All dogs are handled or cared for by Facility staff without liability on Facility’s part for loss or damage from disease, theft, fire, death, escape, injury or harm to person’s, other dog(s) or property by said dog(s) or from other unavoidable causes, due diligence and care having been exercised. Should any daycare dog(s) become ill or seem to be in need of medical consideration, the Facility reserves the right to administer aid and/or to use any available veterinarian. Any expenses so incurred shall be paid by the owner of said dog(s) in addition to other fees listed in this agreement. Owner agrees to pay the rate for daycare in effect by the end of the week in which the pet resided in the Facility. Prices are subject to change at any time, without notice. No daycare dog(s) will be released until all charges are paid in full by the end of the week. Late cancellation fees may also be applied for reservations cancelled after 8 AM on the day of the scheduled reservation. Owner shall remain liable for complete daycare bill, as well as other charges incurred inthe care and maintenance of said dog(s) listed on this contract. It is expressly agreed by the Owner and the Facility, that the Facility liability shall in no event exceed the lesser of the current tangible value of a pet of the same species or the sum of $200 per animal. The owner further agrees to be solely responsible for any and all acts or behavior of saidpet(s) while in the care of the Facility. The Owner of the dog(s) agrees to pay reasonable attorney fees incurred by the Facility in the collections of any daycare services, training, or other charges incurred by the Owner of the dog(s). The dog(s) must be current on vaccinations appropriate for its age; i.e., DHPP, Rabies, Bordetella. Valid proof of vaccinations must be presented before the dog(s) can be dropped off for daycare. Owner also agrees to maintain vaccinations and to keep the Facility appraised of any updates. A negative fecal sample is also required. Pets arriving with fleas/ticks will be bathed at the Owner’s expense. The Facility reserves the right to refuse any dog. Owner agrees that the dog(s) will be picked up by Owner or a pre-approved Owner’s Agent not later than 5:30 PM unless a prior extension is agreed to prior to dropping dog(s) off for daycare. Late charges will be executed, if necessary. Owner agrees that their dog(s) may be video taped, photographed, and recorded. The Facility shall be the exclusive owner to the results and all proceeds of such tapings, photograph, and recordings with the rights, throughout the world, an unlimited number of times in perpetuity, to copyright, to use and to license to others in any manner. The Owner further agrees that the dog(s) may be used in any and all media and in the promotion, advertising, sale, publicizing and exploitation of the Facility. I understand and agree to the above.
